- Chiropractors - please submit an MD, DO, or RN signed prescription here
- Distributors and Wholesalers - please submit your state device distribution license here. The following states require a license for all products labeled RX: Alabama, Arizona, Arkansas, California, Connecticut, Georgia, Idaho, Iowa, Kansas, Kentucky, Louisiana, Maryland, Nevada, New York, Oregon, Pennsylvania, South Carolina, Tennessee, Texas, Utah, and Washington.
- Home Health Care Facilities - If you are a facility that sells directly to patients and if you are located in one of the states below, we will need your prescription medical device license (DME, HME, etc.) on file here. Those states are: Florida, Illinois, Indiana, Mississippi, North Carolina, North Dakota, Ohio, and Virginia.
- Medical facilities, schools, and governmental institutions can purchase freely on the website.
